Service Coordination

A Plus Early Intervention service coordination, on an Initial and Ongoing basis, includes working in partnership with the family to ensure there is communication, collaboration and coordination among all providers involved with the child and family. Service Coordinators provide a quality orientation to the Early Intervention Program and through phone calls, face to face meetings and home visits act as a single point of contact to guide the family through the initial process from referral through eligibility determination. Service Coordinators are also responsible to ensure the implementation of the Individualized Family Service Plans for an eligible child and family; create transition plans; monitor authorized services and protect procedural safeguards/family rights.
